By using this website, submitting information, or requesting services, you acknowledge that you have read, understood, and agree to these Terms and Conditions. If you do not agree, please discontinue use of the website.

3. Scope of Services

Sentry Living Solutions provides supportive services for adults and children with developmental and intellectual disabilities in California, including but not limited to:

• Independent Living Services (ILS)

• Supported Living Services (SLS)

• In-Home Respite Services

• Applied Behavior Analysis (ABA)

• Supported Employment Program

• Personal Attendant Program

• Coordinated Family Support (CFS)

• Self-Determination Program (SDP)

 

All services are subject to written service agreements and applicable Regional Center or funding agency authorizations.

4. Service Inquiries and Assessments

All service inquiries and assessments are based on information provided at the time of initial contact, may be revised following a full intake assessment, are non-binding until a written service agreement is signed, and are subject to funding authorization and eligibility determinations. Services will not begin until a written agreement is executed by all applicable parties.

5. Funding and Authorization

Sentry Living Solutions may assist clients and families with navigating service authorizations through Regional Centers and other funding agencies. However, we are not funding agencies or Regional Center representatives, we do not guarantee service authorization or funding approval, and final determinations are made solely by the applicable funding agency.

6. Payment Terms

Payment terms will be clearly outlined in each service agreement. Services funded through Regional Centers, Medi-Cal, or other agencies are billed directly to the authorized funding source. Private pay arrangements will be detailed in the service agreement. Invoices are due upon the terms stated in the applicable agreement.

7. Scheduling and Service Continuity

Service delivery may be affected by staff availability and scheduling, client health or safety circumstances, Regional Center authorization changes, natural disasters or declared emergencies, and regulatory or compliance requirements. Sentry Living Solutions is not responsible for interruptions caused by circumstances beyond our reasonable control.

8. SMS Messaging Program

Sentry Living Solutions operates an SMS messaging program to communicate with prospective and current clients and their families. By opting in, you agree to receive text messages including appointment reminders, program information, intake updates, and marketing communications.

 

You can cancel the SMS service at any time. Just text "STOP" to 510-422-3959.

After you send the SMS message "STOP" to us, we will send you an SMS message to

confirm that you have been unsubscribed. After this, you will no longer receive SMS

messages from us. If you want to join again, just sign up as you did the first time

and we will start sending SMS messages to you again.

keyword HELP for more assistance, or you can get help directly at

Carriers are not liable for delayed or undelivered messages.

 

Message and data rates may apply. Message frequency varies based on your interaction

with our services. Contact your wireless provider for questions about your plan.

Clients and families who opt in may receive messages including:

• Appointment confirmations and reminders

• Service scheduling updates

• Intake and program information

• Service progress or status updates

• Customer care responses

• Promotional or program notifications

 

Consent to receive SMS messages is not a condition of receiving services from Sentry Living Solutions.

 

We do not share your phone number with third parties for their marketing purposes.

9. Age Restriction

 

By using this website or opting into our SMS messaging program, you represent and

warrant that you are at least 18 years of age. If you are under 18 years old, you

may not use or access our services or opt into our messaging program independently.

Services for clients under 18 are coordinated through authorized parents or legal

guardians.

10. Website Content Disclaimer

Information provided on this website is for general informational purposes only and does not constitute professional clinical, medical, or therapeutic advice. An intake assessment is required to accurately determine appropriate services and supports.

11. Limitation of Liability

Carriers are not liable for delayed or undelivered messages. Sentry Living Solutions is not liable for any delays or failures in receipt of SMS messages as delivery is subject to effective transmission from your mobile carrier.

 

To the fullest extent permitted by law, Sentry Living Solutions shall not be liable for indirect, incidental, consequential, or punitive damages arising from website use, service interruptions, funding agency decisions, regulatory changes affecting service delivery, or acts outside our reasonable control.

12. Intellectual Property

All website content including logos, branding, photographs, text, and design elements are the property of Sentry Living Solutions and may not be copied or used without written permission.

13. User Conduct

You agree not to submit false information, attempt to disrupt website functionality, use this site for unlawful purposes, or transmit malicious software. We reserve the right to restrict access to users who violate these terms.

14. Indemnification
