Coordinated Family Support
Coordinated Family Support (CFS) helps families caring for adults with developmental disabilities by connecting them with resources, providing training, and coordinating services to strengthen support at home. Our goal is to empower families while promoting greater independence and well-being for their loved ones.
Our services are designed to improve equitable access to resources while addressing ethnic and racial disparities. We work closely with families to ensure services are provided in a respectful and culturally responsive manner. At Sentry Living Solutions, our goal is to create a supportive environment where individuals with developmental disabilities can thrive in the comfort of their family home.
Family Support Network aims to:
Keep families together until the person with a disability chooses to live independently
Empower caregivers to meet the many needs of their family member with a disability
Improve support to families while minimizing the need and the cost of out-of-home placement
Allow family participation in integrated leisure, recreational, and social activities
Make a positive difference in the lives of people with disabilities as well as the lives of family members
